MMDA’s Lifetime Whitening Club
Our $99 one-time membership fee includes:
- Custom made whitening trays.
- 2 tubes of whitening gel.
- A handy storage case for when your trays are not in use.
- Free sample of toothpaste
- All instructions (we go over all instructions verbally as well)
- At your routine cleaning you will receive 1 tube of gel at no additional cost. (max of 2 a year)
Only Rules to remain in good standing in MMDA’s lifetime whitening club are:
- You must come in for your routine cleaning once every 6 months.
- In the event you need to cancel, you must speak with someone 48 hours prior to your appointment.
As long as those requirements are met, you will continue to receive a tube of whitening gel at each of your cleanings! As a bonus, this membership also allows members to purchase 2 additional tubes of gel for $25 (1/2 the regular cost of $50) Although most patients are happy with their teeth after using the 2 tubes provided, there is always room to request more. We do offer an in-office boost at a cost of $300 (a 15% reduction from our normal fee.)
We Use Opalescence by Ultradent, a reputable and leading manufacturer of quality dental whitening products! They are Certified Gluten Free, Certified Kosher, and Vegan. There are three flavors to choose from Mint, Melon or Regular.
** If you cancel or miss a routine appointment without 48 hours prior notice your membership will be cancelled. You can rejoin for the $99 fee.
